It is the sole responsibility of the patient to contact their insurance company to verify their benefits/coverage for anesthesia administered for the procedure. The Center does not verify insurance coverage for anesthesia provided during the procedure.

Physician services provided in the Facility are not included in the Facility’s charges.

Physicians who provide services in the facility may or may not participate with the same health plans as the Facility.

Patients should check with the physician arranging for the Facility services to determine the health benefits in which the physicians participate.

Patients should contact their carrier for further consultations on those costs.
